This is Anders Weidemann's vanity card.

Logo: On a black background, we see an extreme closeup of an eye opening in black-and-white, with its pupil in brown, all inside a boxed rectangle. As this is happening, the company name appears below it in white. The entire logo has an videotaped-like feel to it.
Technique: Live-action and videotape effects.
Music/Sounds: None.
Availability: Seen on Interrogation on CBS All Access (later Paramount+).
Legacy: The eye opening may startle some first-time viewers.
