This is the vanity plate of Steve and Nancy Carell.

Visuals: On a black background, we see an old boxed film footage of a young Nancy Carrell with her father Robert riding on a carousel, with a building behind them. Half a second later, the image freezes and shrinks, revealing "CAROUSEL".
Technique: Live-action and 2D computer animation.
Audio: A 9-note xylophone tune followed by a 1-note violin tune.
