Background
Human Head was founded in 1997 and located in Madison, Wisconsin. The company participated in Gathering's Blair Witch project, where three game parts were created by different developers. They didn't use their logo in every game.
1st Logo (July 30, 2001)
Logo: We see a processing plant assembly line, delivering metal shields which in fact are Human Head logos, depicting the upper part of a head in goggles and the name below. One of the shields is finished and falls to the floor, with us following it and fading into the regular logo.
FX/SFX: The shield, the factory.
Music/Sounds: Machine sounds, beeps and wheezing.
Availability: Seen on Rune (only) for PlayStation 2.
2nd Logo (2004)
Logo: The logo is planted over a Western Wanted poster, which offers $20000 for "any who wear this brand". Then it says: Human Head Studios - CLG Wiki

3rd Logo (2006)
Logo: Starting on an alien planet, we see strange rocks around, and a gas giant planet in the sky. Then we zoom out of the human head's eye, and the name appears below, vibrating constantly. The red line is drawn under the name.
FX/SFX: Animation of the alien planet and vibrating name. Nice CGI.
Music/Sounds: Wind sounds.
Availability: Seen on several trailers for Prey, but not on the game.
4th Logo (2006-present)
Logo: TBA.
FX/SFX: The camera tilting, the the screen.
