Background
This is the vanity card of Paul Attanasio and Katie Jacobs.

Logo: Set on a stage, we see a small figurine of a ballerina and a man dancing and twirling around. Suddenly a human foot wearing a shoe stomps on the couple, crushing it. Then, "HeeL & Toe" (in white, and in a strangely-arranged Times font) appears next to the shoe.
Variants:
- A short version exists on House, M.D.
- The logo would come in either the standard 4:3 version (seen on full-screen prints) or the widescreen 16:9 version.
Technique: Live-action.
Music/Sounds: A music box rendition of "Für Elise" plays as the dancing ballerina turns and a "stomp" sound as the couple is crushed. A squishing sound is also played as the foot comes down.
Music/Sounds Variant: ABC airings of Gideon's Crossing use a generic theme and leaves the squishing sound intact.
Availability: Common. First seen on the series Gideon's Crossing and also appeared on Century City and House, M.D.
