Logo: We see "JAVN", wiping in, with a shadow, the shadow shrinks and expands, "JAVN" shrinks, then the symbol forms, "Japan Audio Visual Network" wipes in.

FX/SFX: The wiping in, the shadow, the shrinking, the symbol forming.

Music/Sounds: First, there's silence. When the symbol appears, we hear the sound of the wind with a loud bang when the animation ends. Sometimes, the bang is omitted.

Availability: Seen on the label's japanese VHS releases, such as the Space Battleship Yamato anime.
