Background
This is Daniel Zelman, Glenn and Todd A. Kessler's vanity card.

Logo: We start off with the white background, then ends up quickly turning into the gray, thin rectangle placing in the center as the words appear, on top, "KZK", and on the bottom "PRODUCTIONS." Then, the star behind the first K appears.
FX/SFX: The words, and the star.
Music/Sounds: The sound of the bowling ball rolling followed by the pins knocking down.
Availability: Seen on Damages and Bloodline.
