This is Pauly Shore's vanity card.

Logo: Below the 3 Arts Entertainment logo, we see a cheaply-drawn moon surface in space, with a guy's head (likely Pauly Shore himself) tilting slightly left to right in a white rocket with three purple circles underneath. On the left side is the large white text "landing patch" with the spaced out text "PRODUCTIONS" underneath it to the right out of position.
FX/SFX: Just the guy's head tilting slightly.
