This is Pauly Shore's vanity card.

Logo: Below the 3 Arts Entertainment logo, we see a cheaply-drawn moon surface in space, with a guy's head (likely Pauly Shore himself) tilting slightly left to right in a white rocket with three purple circles underneath.
