This is Amy Brenneman's vanity card.

Logo: On a white background, we see a stylised drawing of a woman with only one of her eyes shown, who is designed in a plant-like fashion with green stems and large orange petals. A small purple flower is at the end of the stem. Underneath the flower is "Molly Bloom" with "Productions" underneath both in a typewriter font. The logo is next to the Reveal Entertainment logo.
