Logo: On a black background, we see a white silhouette of a child jumping towards a basketball net with a dark colored basketball. The kid manages to make a slam-dunk. (thus the company name) As it passes through the net, the basketball suddenly transforms into the words:

SLAM
DUNK

After this, "PRODUCTIONS", which is written in a fancier font and colored gold, slides in from the left and positions itself under the company name. There's also a line seen below "SLAM DUNK".

FX/SFX: The boy making the dunk, the ball "transforming" into the words.

Music/Sounds: The sounds of the wind blowing when the boy is in the air, a "thud" when the ball hits the next, followed by the sounds of a cheering crowd which end abruptly (due to it cutting to the Dick Clark Productions logo).

Availability: Extinct. Was only seen on Double Up and Mad Libs.
