This is the vanity card of David Windsor and Casey Johnson.

Logo: Against a blue-black dusk sky with the Los Angeles skyline at night, we see a bowling sign that has three pins colored in yellow and a bowling ball on the top, which blinks twice, followed by the company name in different red and blue fonts respectively doing such. The logo zooms in a bit.
Variant: Starting with Not Dead Yet, the zooming effect and moving clouds in the sky are omitted.
