Background
Shortly after VCI and BBC Video merged to form 2 Entertain, Channel 4 ended their partnership with VCI and began self-distributing their DVD releases.
1st Logo (2005-2013)

2nd Logo (2013-2016)

Visuals: We see pieces of the Channel 4 logo (with footage from the 2004 idents in them) form together on a grey background, with the complete result forming the logo with the left block replaced with "DVD". The logo then rotates.
Technique: CGI.
Audio: A tribal theme with a clang at the end.

Final Note
As of 2016, Channel 4 licenses its DVD and Blu-ray releases to Dazzler Media, Acorn Media, among other companies.
