Background
Sledgehammer Games is a subsidiary of Activision founded in 2009 by Visceral Games alumni Glen Schofield and Michael Condrey, formerly of Dead Space fame.
1st Logo (November 8, 2011)
Visuals: On a green and white radio wave/heartbeat monitor background, "SLEDGEHAMMER GAMESTM" in a giant metallic font fades in. It shortly dissolves out.
Technique: CGI.
Audio: The sounds of the monitor.
Availability: Seen on Call of Duty: Modern Warfare 3.
2nd Logo (November 4, 2014)
Visuals: After the Activision variant on the game, "SLEDGEHAMMER GAMESTM" in a giant metallic font forms from streaks and "slams" into place. A flare on the "D" goes down. Shortly after, the logo "flashes" into that of the game proper, and fades out to the beginning.
Technique: CGI.
Audio: A hammer noise for the text, and the rumbling of the background throughout.
Availability: Seen on Call of Duty: Advanced Warfare.
3rd Logo (November 3, 2017)
